The biggest mistake IMHO is that the US thought their soldiers would be welcomed as liberators, that by bringing the US way of life to them, they would win the hearths and minds of the people of Iraq and Afghanistan. Big mistake. The US soldiers were not perceived as liberators, they were perceived as an occupational force. Which of course they were.
IMHO, this did not start with George Bush the 2nd. It started with George Bush the 1st who stationed troops in Saoudi Arabia during Gulf war I. That was perceived as an invasion of holy ground. Even worse, those troops were not withdrawn when Saddanm was driven out of Kuwait. This eventually led to the 9/11 attack. The US brought it on themselves by systematically ignoring the sensitivities provoced by their global 24/7 military presence.
