One of the reasons I choose the lowest speed that my ISP offers. 40 Mbps is high enough to ever so often have the upload at the other end be the bottleneck...

For you, yes. For adsl there is a strong correlation between distance to the DSLAM and obtainable speed. Cable is not very sensitive for distance. OTOH, the last mile on the cable is shared with many others. The last mile of copper pair (or quad) for dsl is all yours.

We tried to get fibre here last year when the sewer system in the street was renovated. None of the providers was interested... :-(
