Same here, but you must also show an ID. The people at the voting office have e alist of all persons qualified to vote there. The names and Personal Identification Number must match with the list.
For voting by proxy there is of course a procedure. On the back of the voting notice there is a space for filling in the name and Personal Identity Number and the the person you give a mandate to submit the vote. Plus signatures of both parties. At the voting office one must present the voting notices and a (copy of) an ID of the other person together with one's own papers. The data of the person you vote for must also match the list.
Any voter can submit a maximum of two proxy votes.

I would not be surprised if in Germany there is a procedure similar to that in The Netherlands.
