Long ago on some busy multiuser BBSes it was certainly possible for two users to generate the same MSGID serialno if the algorithm used was based on C's time_t (seconds since 1970-01-01 for most implementations) and they users posted a message at the same time. Potentially this could also happen with QWK mail uploads.
How often this happened in real life, I'm not sure. Back then I don't recall it being common for mail processors to do dupe detection based on MSGID alone. If they did I suspect it was mitigated somewhat by dupe detection usually being done on a per-area basis, which I think is still the case today.
