I had a discussion about that with an ISP.
They were looking into PCP (Port Control Protocol). Which is the dumbest thing ... a way to get some port forwards on carrier grade NATv4.
I proposed just the same as you ... It makes much more sense, you can charge additionally something for the v4 address, and it is less hassle for the ISP and the customer ...
